Addressing Social Determinants of Health and Targeted Risk Factors

Helping Busy Healthcare Professionals Efficiently Implement SEEK

SEEK (Safe Environment for Every Kid) is an evidence-based psychosocial prevention and intervention model for primary care practices serving children. It assists healthcare professionals to identify and address targeted risk factors for child abuse and neglect in families with young children.

A Comprehensive Approach for Targeted Psychosocial Problems

By using SEEK, within NavWell, to implement the SEEK approach, practitioners can strengthen families, support parents and parenting, promote children’s health, development and safety.

Key Features

Facilitates the Use of SEEK

Prior to a child’s appointment, a caregiver can complete SEEK’s Parent Questionnaire-R (PQ-R) electronically at home.

Provides Valuable Resources for Caregivers

SEEK resources and guided discussions can be accessed in the primary care provider’s office before or during a child’s checkup. Customized caregiver plans and handouts are easily printed as a take home resource for the family.

Secure and HIPPA Compliant

SEEK, delivered via the NavWell software application, is fully HIPAA compliant and Advanced Metrics will work with you to complete a Business Associates Agreement to fulfill all obligations of covered entities.

Supports Meaningful Use Reporting

SEEK data supports “meaningful use” reporting requirements by transmitting important related data to the client’s Electronic Health Record, including data that addresses Clinical Quality Indicators associated with National Quality Strategy domains.

Quality Improvement and MOC 4 Data

SEEK data can be used for the SEEK MOC 4 Activity, which is approved by the American Boards of Pediatrics and of Family Medicine.
